Healthier Living
and
Diabetes Empowerment Education Program

The Healthier Living Program and Diabetes Empowerment Education Program (DEEP) are six-week workshops focused on Chronic Disease Self-Management and Diabetes Empowerment Education. They are available at no cost to the community in a variety of languages (English, Spanish, Chinese, Tagalog, and Russian) at various centers throughout San Francisco.

Healthier Living

Healthier Living is a six-week workshop for persons with ongoing health conditions that affect their quality of life, such as arthritis, diabetes, stroke, high blood pressure, heart disease, obesity, cancer, lung disease, asthma, chronic fatigue, chronic pain, Parkinson’s, depression, Hepatitis B or C, HIV, and others. Workshops are at no cost to the participants, and conducted in different languages. Groups meet once a week for 2.5 hours. Healthier Living was designed by Stanford University, and it is facilitated by two peer co-leaders in a small group participatory setting.

The objectives of this program are to:
• Empower and educate adults with chronic conditions to self-manage their health.
• Instill confidence in participants by using the learning philosophy of self-efficacy.

Topics covered include:
• Setting and achieving goals
• Problem solving and Decision making
• Getting better Sleep
• Preventing Falls
• Improving communication with medical providers and loved ones
• Using Mind-Body tools including: better breathing and body awareness
• Becoming a partner with their doctor to manage their heath
• Learning the importance of physical activity and healthy eating
• Understanding medication usage
• Participants learn, share, build bonds with others and have fun.

Diabetes Empowerment Education Program

Diabetes Empower Education Program (DEEP) is a six-week workshop series facilitated by a trained health educator, which provides information and hands-on activities to help manage diabetes. This program is open to individuals who are diabetic, pre-diabetic or are interested in gaining knowledge about the condition. Workshops are at no cost to the participants, and conducted in different languages. Groups meet once a week for 2 hours. DEEP was developed by the Midwest Latino Health Research Training and Policy Center at the University of Illinois at Chicago.

The objectives of this program are to:
• Decrease diabetes risk factors, including obesity and hypertension
• Increase knowledge of diabetes and its risk factors
• Increase self-management skills to manage stress, and to facilitate short and long term behavioral goals.

Topics covered include:
• Understanding the human body
• Understanding diabetes and its risk factors
• Improving physical activity
• Managing health through nutrition
• Identifying and preventing diabetes complications
• Communicating with medical providers and managing medical care
• Managing stress; and communicating with family and friends.

Voluntary contributions are welcomed and encouraged but no one will be turned away regardless of donation.
